They work collaboratively with clients, families, service providers, and other stakeholders to ensure that individuals receive the support they need to lead fulfilling lives.
Our Social Workers can support with:
- Assessment and Planning: Social workers assess the needs and goals of individuals with disabilities and collaborate with them to develop personalized plans. These plans outline the necessary supports and services required to achieve the desired outcomes.
- Advocacy: Social workers advocate for the rights and interests of individuals with disabilities within the NDIS system and the broader community. They may assist clients in navigating complex processes, accessing entitlements, and asserting their rights.
- Emotional Support and Counseling: Social workers provide emotional support and counseling to individuals with disabilities and their families. They help them cope with the challenges associated with disability, navigate transitions, and build resilience.
- Linkages and Referrals: Social workers connect individuals to relevant services and resources in their community. This may include healthcare providers, support groups, educational programs, vocational training, housing assistance, and recreational activities.
- Capacity Building: Social workers empower individuals with disabilities to develop skills, knowledge, and confidence to participate fully in society. They may provide training in areas such as communication, decision-making, problem-solving, and self-advocacy.
- Family Support and Education: Social workers offer support and education to families of individuals with disabilities. They assist families in understanding the NDIS process, navigating family dynamics, accessing respite care, and building networks of support.
